Everyone needs to read this book. Everyone at some point in his or her life is going to be involved with what Reznik calls the "Health Care Machine." This book should be read before you visit your doctor, have any procedures done, or make any medical decisions.

"The Secrets of Medical Decision Making" is not completely about secrets, but rather expresses the concerns the medical community now has about treating patients, and how those concerns have become less about the patient, and more about fear of being sued by patients, as well as dictated to by pharmaceutical and insurance companies. Patients are often subjected to many unnecessary tests and procedures because doctors feel they must cover all the bases just in case they are sued. Reznik gives several examples from his own experiences as a doctor where he and his colleagues have had to perform tests or procedures that were unnecessary and in some cases, gave patients unneeded extra stress or discomfort all in the quest to be thorough to protect the hospital and the medical profession.

Much of "The Secrets of Medical Decision Making" is just about common sense. Patients have a responsibility to use common sense and to keep their own best interests in mind when being treated by a doctor. In an age where surgeries are often outpatient and we are bombarded with information about new miracle pills that will resolve problems, patients are given false expectations that medicine can resolve nearly every health issue; consequently, patients are ready to sue when something goes wrong, when truthfully, they do not ask the appropriate questions about risks involved with procedures, and doctors do not provide their personal opinions unless asked because of restrictions imposed upon them by the medical and insurance systems.

What I found most interesting about "The Secrets of Medical Decision Making" was Reznik's elaboration on the many unnecessary procedures and tests given, including colon and breast cancer screening. He takes a hard look at the statistics medicine uses to suggest that pre-screening can prevent cancer or increase lifespan. Reznik states patients must be more pro-active in asking their doctors about the necessity of many tests and how likely it is they will actually have the disease for which the screening is being performed. Patients also need to trust their own decisions because they know their own bodies better than their doctors, and they know what will make them most comfortable.

While at times, I found Reznik's discussion a bit repetitious, I appreciated the summary he placed at the end of the book, breaking each of his main points into a paragraph with a subtitle so a person can quickly refer to it and remember which questions to ask before a visit with a physician.

"The Secrets of Medical Decision Making" is a book everyone will benefit from. Patients need to realize that modern medicine does not have the cure for everything-in fact, it can cure very little, and therefore, the patient must be largely responsible for his medical care and decisions. This book will help the patient have more control over his treatment and give him useful questions to ask his doctor. I intend to tell everyone I know in the medical professions, as well as everyone I know with health issues, about this book. Patients will save themselves many hours of wasted time, money and discomfort simply by spending a few hours reading "The Secrets of Medical Decision Making."

At -25 per pair, the cost of Totes Isotoner slippers is higher than most other brands. Still, the wear and tear is minimal, my feet stay warm, and I get to take out the trash [oh, joy!] without having to switch over to shoes. I doubt that you will find a best pair of men's slippers for this price.

History

In the 1780's Brighton had begun growing away from its roots as a Fishing Town catering for a more upper class visitor to use a seaside retreat due to its close proximity to London. One of these visitors was the Prince of Wales, George. Who purchased a small beach hut. The prince had been advised by his physicians to visit Brighton to benefit from the climate of Brighton, as well as to try out the sea water remedies that had been suggested by Dr Richard Russell who lived in nearby Lewes.

Brighton was George's ideal city. It had everything he loved; fashion, arts, architecture and good living. It was his love of the high life that enabled him to rack up massive debts. In 1787 Pariliament agreed to wipe out the debts of George and to increase his income. This is where the real story of the Pavilion begins.

George hired the well respected Architect Henry Holland to help shaped his small beach hut into a palace fit for a king. The first version of the Pavillion that we know today was a villa which went by the name of The Marine Pavilion. George set about decorating the villa to his own unique style; importing many goods from China including furniture as well as hand painted wallpaper. In 1808 a stable was added which contained housing for 62 Horses.

In 1811 due to the declining health of his father George III, the prince was sworn is as prince regent, effectively making him King of the country. Over time the prince realised that this villa was too modest in size for the extravagant parties that he wished to throw, so he hired John Nash to begin developing the small villa into the large palace that we know today. This was done by John Nash building a cast iron frame around the original building, which gives the building the unique look that it now has.

George became King in 1820. However due to these extra responsibilities coupled with ill health after the completion of the interior in 1823 he was only able to make 2 more visits before his death.

King William IV was the successor to George upon his death and continued to visit the Royal Pavilion. However due to being a married man it did not have the space for him and his ever growing family. He therefore had to build 4 additional buildings to the Pavilion, most of which have now been demolished.

Upon Williams death the next in line was Queen Victoria. Who first visited the Pavilion in 1837. However the style and size of the palace was not to her tastes. She last visited the palace in 1845. After this Brighton Commissioners and the Brighton Vestry campaigned to the government to sell the palace to town of Brighton. Its sale was agreed under Brighton Improvement (Purchase of the Royal Pavilion and Grounds) Act 1850 for £53,000. As the Queen assumed that the building would be demolished she stripped it of all its interiors using them for other royal residences. She however did return many of the items in 1864 & 1899 although still much of what is on display is a replica.

During World War I the palace was used as a hospital for injured Indian Soildiers.

In 1920 restoration work was carried out; which helped reverse some of the damage caused by being used as a hospital. Queen Mary also returned a lot of the decorations which had been stored at Buckingham Palace. This work has continued ever since.

The Pavilion today draws in over 400,000 visitors a year, making it the most popular tourist attraction in Brighton.

The Rook was originally symbolizing of a chariot and the word itself sounds remarkably familiar to the Persian word for "chariot." These Persian war chariots were often heavily armored with fortified stone work, giving the Chess piece the image of a mobile building. The design eventually reshaped itself into a turret and the modern Rook was born. Knights are usually represented by horses and are rather straightforward in their history and appearance. They never have had drastic meaning changes or appearance.

Bishops seem to generate from the Staunton Chess set. They are given a tall hat, much like a Bishop's mitre. This form, however, also pays homage to the original form of the Bishop, with the traditional deep groove also symbolizing the tusks of the elephant that the piece initially represented. The Queen, however, has had the most transformation over time. Originally only being allowed to move one space diagonally and to make a jump like a Knight only once in a game, by 1600, the piece was given the powers it is granted today. Additionally, the King was changed from a General to keep with the modern court theme. Clinical Presentation:

It is considered as a superficial dermatitis, that is usually seen in atopic [prone to allergy] children and presents with dry-scaled, light colored patches on the face - especially around the mouth. Due to its color it seems accentuated during the summer. It might itch during the early stage of appearance.

It is most likely to be mistaken with vitiligo or leucoderma, where there is a complete absence of the dark pigmentation. The pityriasis alba lesions do not usually have a well-defined borders [like in vitiligo] and they do not coalesce [as in tinea versicolor].

Treatment:

Ideally treatment is not necessary for pityriasis alba, but one should consult a physician to rule out the other causes of hypopigmentation, which might need treatment. Anti-fungal ointments have shown improvement in some patients.

"So I am opening with a Magoo moment. Dad decides to take a shower. He always announces his intentions because I need to make sure he gets in and out safely. It's part of our system. However, coming to the front door buck-naked while we have guests on our back porch is not part of our system. He opens the door and says he needs some help. This could be anything from "I can't find a bar of soap" to "I fell and there is blood all over the bathroom." Well, it was neither. I followed (without his walker) naked Magoo to his bathroom, and when we got there, he said he couldn't get the sprayer back on the holder! I needed scuba gear. There were two inches of water on the bathroom floor. Everything was drenched...even Lilly (his dachshund) who was stationed behind the bathroom door looking for a snorkel. The toilet bowl looked like a Jacuzzi. The vanity was a small pond. All we needed to do was add fish. Okay...do I look like Stanley Steemer...looking for alpaca poop to scoop? Or do I just need to go get the wet vac? I have renamed his bathroom Lake Magoo. Never install a shower massage in a handicap-equipped shower stall for a ninety-seven year old. Hell, I don't know what was worse...Lake Magoo or naked Magoo. OMG!"

Trust me; Leilani will have you laughing on practically every page. Nor is she hiding the pain behind the laughter-instead she openly details the painful and embarrassing moments, but her nature is to laugh at life regardless. I imagine few people have her sense of humor and courage when faced with such a situation, but I also imagine that many people will find a new perspective from which to deal with their own battles, whether they are cancer or other obstacles, while reading this book. If you know someone with cancer, this book will be the perfect gift since we know what they say about how laughter is the best medicine. This book is rich in a variety of experiences ranging from doing emergency care on an airplane to helping overweight people out of their homes, and an emergency services worker experiencing an accident and then seeing things from the patient's side as her co-workers care for her. Mayo shares her own experiences throughout, but she also shares the stories of co-workers, her daughter who was inspired to follow in her mother's footsteps, and numerous other first-person accounts of helping in an emergency.

While "Confessions of a Trauma Junkie" has numerous telling and moving stories, what I appreciated most was the humor. The humorous passages actually made me better understand how emergency services personnel respond to the most difficult situations, the boredom they have to deal with, and the need for humor as a coping mechanism. The workers also do not always receive the appreciation and respect they deserve. They become rightfully irritated when treated like servants, when they are threatened with lawsuits, or when the lazy try to take advantage of them, refusing even to sit up by themselves because a worker can pick them up. While Mayo's experiences occur in the greater metropolitan Detroit area, and a larger economically and socially challenged population exists there, I imagine emergency services workers need to deal with rude and inconsiderate patients constantly, whether in rural or metropolitan areas.

The humor in this book was so rich that I would recommend it to any emergency services personnel simply as a way to cope and relieve stress with a good laugh. Among the many humorous passages, one of my favorites was the response received from a patient when the admitting nurse asked for her name:

I'm too sick to talk. Ask my husband what my name is...can't you see I'm sick? What's wrong with you people, asking me questions when it's obvious I can't breathe? You want to know anything, you ask my husband. I don't have enough breath to answer your questions. You supposed to be a Nurse, you should know that.

Many similar stories are told regarding simple questions asked to patients, as well as shenanigans by patients that range from urinating on the hospital walls to showing up for free pregnancy tests and free meals, and another favorite, the man who complains he's having a heart attack, and when the nurse diagnoses that he is not and asks him to wait, decides to go out for a smoke.

Mayo also tells many stories of rewarding experiences, most notably of going to help people in Mississippi after Hurricane Katrina. Thank you for whatever you did."

I say, "Thank you, Sherry Jones Mayo, for writing this important book so you and your co-workers who work twelve hour shifts, stay overtime, wear down your own bodies caring for ours, and put up with situations no one should have to, will get the recognition you deserve for everything you do for the rest of us. Thank you for caring for us, even when we are not desirable patients. I don't know how you can do it-it's not a job I could do-but I won't forget what you and your co-workers go through everyday next time I need care."

By the end of the 18th century between 150,000 and 200,000 watches a year were being produced by this system, satisfying the national need for accurate timekeeping as the industrial revolution took hold.

The Lancashire sub-contracting system allowed the production of watch movements at such low prices that by the end of the 18th century, the Lancashire manufacturers were supplying most of the great watch firms in London, Coventry and Liverpool. All that these firms needed to do was to make or source their own case and dial, and then assemble the watch.

Thomas Russell joined this hive of activity in 1848 when he moved his business as a watch manufacturer to Slater Street in Liverpool. The city was a major seafaring port and the manufacture of ships' clocks and chronometers became an important revenue stream for the business.

Thomas Russell's father, also named Thomas Russell (1780-1830), the founder of this watchmaking dynasty, was born in Eskdale a small village in Cumberland. He served his time in watchmaking in New St. Broughton-in-Furness Lancashire under William Bellman, he then served his journeyman time with William Wakefield in Market St Lancaster where he later started a business of his own in the same street.

He had two sons; one named Thomas was married to Mary in 1831. They also had two sons, Thomas Robert (1833-1894) born in Lancaster and Alfred Holgate Russell (1840-1893). In about 1840 the family moved to Halifax setting up a watchmaking business in Lord St. It was here that Alfred was born.

By 1848 the family had moved once more and records show that Thomas Russell was a watch manufacturer with premises at 20 or 22 Slater Street, Liverpool and later at number 32 in the same street. It was here that Thomas Russell became arguably Liverpool 's finest watchmaker and the business produced quality watches and clocks, including the celebrated Russell Hunter pocket watch. Thomas Senior and his oldest son Thomas Robert were granted a Royal Warrant by Queen Victoria indicating their rapid progress in watch manufacturing.

Around 1859, Thomas handed over control of the business to his sons Thomas Robert and Alfred Holgate and the company changed its name to Thomas Russell & Son. Following Thomas Russell's death in 1867 the business was divided into two; the trade side continued under the same name and was run by Alfred and Thomas ran Russells Limited. The retail business became importers of Swiss watches and music boxes.

By 1877 the company had moved the business once more, this time to Cathedral Works, 12 Church Street, Liverpool, with additional offices at Piccadilly in London and Toronto, Canada. It was now known as the Russell Watch and Chronometer Manufactory and was listed in 1880 as "watch and chronometer manufacturers and machine made keyless lever and jewellery merchants" and additionally, "by Appointment to Her Majesty the Queen and HRM the Duke of Edinburgh and the Admiralty".

After Queen Victoria's death, Thomas Russell still signed their watches "Makers to Queen Victoria" even though officially the warrant had ceased with the Queens death. This was tolerated for a time before they removed this from their watches.

Following the deaths of Thomas Robert and Alfred Holgate, Alfred's son Bernard Holgate Russell and his cousin Thos Townsend Russell took over the company and the name of the business was changed in 1894 to Russells Limited. From this date it appears that they continued as retail jewellers with several branches in Liverpool and, by the early 1900's, Manchester and Llandudno as well.

Bernard married and had a son Thomas Graham (1906-1999). In 1915 Bernard and Thos Townsend Russell invited Joseph Wright to become a fellow director of Thos Russell & Son. Joseph had extensive trade knowledge, travelled extensively and had business contacts in Switzerland and working experience with the famous American Illinois Watch Case Co.

The sons of these directors all seemed to have worked in and run the business in later years. During WW2 Joseph Wright kept the firm going despite wartime shortages of materials and men until the sons returned from the war.
